<abstract xml:lang="en"><p>The article presents a comparative analysis of structural healthcare models in the world's largest economies (China, USA, India, Russia, Germany, Japan) and leading countries in industry rankings (Australia, South Korea). The article aims to identify fundamental directions of divergence among national systems regarding resource provision and financing mechanisms. The methodological framework is based on a comparative structural analysis of the WHO and UN's data for the period 2010−2023.
The authors argue that the key factor defining efficiency differences is not the absolute volume of funding, but the system's architecture. The article confirms the existence of a “financing paradox”: extremely high expenditures (the US model) without universal coverage do not translate into increased life expectancy. The article identifies two polar models of workforce and infrastructure provision: the “hospital-physician” model (Russia and China), characterized by a focus on inpatient care and a deficit of nursing staff (doctor-to-nurse ratio of approximately 1:1.4), and the “ambulatory-delegating” model (Western countries), distinguished by a developed nursing institution (ratio of 1:3 and higher). It is concluded that improving the efficiency of the Russian system requires structural balancing of the workforce composition rather than extensive capacity expansion.</p>

<trans-abstract xml:lang="ru"><p>В статье представлен сравнительный анализ структурных моделей здравоохранения стран с крупнейшими экономиками мира (Китай, США, Индия, Россия, Германия, Япония) и государств-лидеров отраслевых рейтингов (Австралия, Южная Корея). Цель работы заключается в выявлении фундаментальных направлений дивергенции национальных систем в части ресурсного обеспечения и механизмов финансирования. Методологическую основу исследования составил сравнительный структурный анализ данных Всемирной организации здравоохранения (ВОЗ) и ООН за период 2010−2023 гг.
Авторы обосновывают, что ключевым фактором различий в эффективности является не абсолютный объем финансирования, а архитектура системы. Подтверждено существование «финансового парадокса»: сверхвысокие расходы (модель США) без обеспечения всеобщего охвата не конвертируются в рост ожидаемой продолжительности жизни. Главным научным результатом стало выделение двух полярных моделей кадрового и инфраструктурного обеспечения: «госпитально-врачебной» (Россия, Китай), характеризующейся упором на стационарное лечение и дефицитом среднего медперсонала (соотношение врач/медсестра около 1:1,4), и «амбулаторно-делегирующей» (страны Запада), отличающейся развитым институтом сестринского дела (соотношение 1:3 и выше). Сделан вывод, что для повышения эффективности российской системы необходима структурная балансировка кадрового состава, а не экстенсивное наращивание мощностей</p>
